Before your visit to Haven Lake Animal Hospital, you are able to fill out our online forms at your convenience. This step will help us gather the information needed to see your pet and save you time upon arrival. All forms will be emailed directly to us and attached to your records. Please allow 24 hours for prescriptions to be filled using our RX Refill form. A member of our staff will call to confirm when your refill is ready for pick-up.
